Consequently, compliance with regulations governing the protection of sensitive biometric data, including the European General Data Protection Regulation (GDPR)[<strong>1<\/strong>], has become an increasingly important issue.<\/p>\n\n\n\n<p>Conventional anonymization (pseudonymization) of DICOM metadata alone does not sufficiently protect patient identity, since the facial morphology reconstructed from CBCT data may still enable personal identification. Facial de-identification (defacing) has therefore emerged as an important complementary strategy for protecting patient privacy in volumetric imaging.<\/p>\n\n\n\n<p>Compared with conventional CT and MRI, however, effective defacing methods for CBCT remain limited. This is mainly due to the intrinsic characteristics of CBCT imaging, including increased noise, lower soft-tissue contrast, beam-hardening artifacts, and the absence of reliable skin segmentation. Although synthetic CT approaches have been proposed, they were developed for different clinical purposes and generally do not address CBCT facial de-identification[<strong>2-6<\/strong>].<\/p>\n\n\n\n<p>The main technical challenge is therefore straightforward:<\/p>\n\n\n\n<p><strong>How can facial morphology be removed while preserving the complete internal anatomy required for implant planning, AI development, or research?<\/strong><\/p>\n\n\n\n<p>While AI-based defacing algorithms continue to evolve, practical geometry-based alternatives can already provide efficient solutions.<\/p>\n\n\n\n<p>This presentation introduces two geometry-based defacing workflows for CBCT that rely exclusively on open-source geometric tools. This curve defines the cutting boundary separating the facial soft tissues to be removed from the anatomical structures that must be preserved, including the jaws, teeth, mucosa, mandibular canal, and other clinically relevant structures.<\/p>\n\n\n\n<p>After executing the algorithm, all voxels located anterior to the curve are reassigned to the background (air) value, producing a defaced CBCT volume while leaving the internal anatomy unchanged.<\/p>\n\n\n\n<p>The resulting dataset can immediately be used for implant planning or exported as a new anonymized DICOM series.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Algorithm<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Read the Open Curve.<\/li>\n\n\n\n<li>Convert curve points into RAS coordinates.<\/li>\n\n\n\n<li>For each axial slice, identify the closest curve point.<\/li>\n\n\n\n<li>Use the curve as a dynamic geometric clipping boundary.<\/li>\n\n\n\n<li>Replace all voxels located anterior to the boundary with background values.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Principle<\/h3>\n\n\n\n<p>The open curve is not merely a drawing spline. The operator manually outlines the patient&#8217;s head across the axial, coronal, and sagittal views, generating a closed segment enclosing the entire anatomy.<\/p>\n\n\n\n<p>The Hollow effect (Outside 3\u20135 mm) is then applied to create a thin shell.
